Registration Information
Please use this Doodle poll to register for your preferred time slot. Choose only one of the available slots.

Your deadline to respond is by 4:00pm of October 15th. Note that you must indicate your intention to participate in advance. This is critical to ensure that adequate clinic staff and vaccination doses are provided. The University Health Center indicated that walk-ins will not be accepted. Each time slot accommodates 20 patients. If by any reason you cannot make the time you selected, please go back and delete your participation.

You must establish a record with the Health Center and provide an acceptable form of payment by the day of the clinic in order to receive your flu shot at the on-site clinic. The flu shot can also be paid out of pocket for $44.00.
