Flu Shot

The School of Public Health will host its Sixth Annual Flu Shot Clinic in the Faculty/Staff Lounge (room 0393) on Tuesday, October 22nd from 8am to 3:30pm. 
 
Last year 159 SPH faculty, staff, and students were vaccinated, consistently keeping the number above 150 for the past 4 years. Let's beat our marks and increase the number this year!
 
How to register
  • Click on the link to the Doodle poll to register for your preferred time slot. Each time slot accommodates 20 patients, please choose only one of the available slots. Your deadline to respond is by 4:00pm on October 18th. Note that you must indicate your intention to participate in advance, this is critical to ensure that adequate clinic staff and vaccination doses are provided. The University Health Center indicated that walk-ins will not be accepted.   • If you have never been to the University Health Center, you must call the registration desk at 301.405.7440 to register with them or email a picture of your insurance card to uhc-registration@umd.edu. Provide your UID number, home address, and local telephone number. Tell them you are registering because of an upcoming flu clinic. You must establish a record with the Health Center by the day of the clinic in order to receive your flu shot at the on-site clinic. 
Getting your flu shot
 
  • You must provide one of the following acceptable forms of payment at the time of your service: 
 
A. For Faculty and Staff: the vast majority of health insurance carriers cover the full cost. Submit a photocopy of your insurance card (front and back) and your UID card. If you are not the subscriber, please specify who the subscriber is (spouse or parent), and write the name and address of the subscriber. The Health Center is in network with CareFirst BCBS PPO/EPO, United Health Care PPO/EPO, Aetna PPO/EPO, Cigna PPO/EPO, or the SHIP insurance. HMO, Kaiser Permanente, Medicare, and out-of-state Medicaid are not accepted.
 
B. For Graduate Students: submit a photocopy of your insurance card (front and back) and your UID card. If you are not the subscriber, please specify who the subscriber is (spouse or parent), and write the name and address of the subscriber. The Health Center will submit the claim on your behalf to CareFirst BCBS PPO/EPO, United Health Care PPO/EPO, Aetna PPO/EPO, Cigna PPO/EPO, or the SHIP insurance. HMO, Kaiser Permanente, Medicare, and out-of-state Medicaid are not accepted. 
 
C. For Undergraduate Students: submit a photocopy of your insurance card (front and back) and a copy of your UID card. If you are not the subscriber, please specify who the subscriber is (spouse or parent), and write the name and address of the subscriber. The University Health Center will submit the claim on your behalf, otherwise you will be billed through the Bursar’s Office. 
  • Recommended attire: A loose sleeve or T-shirt to provide easy access to the injection site (upper arm). 
There will be ice cream served from the Maryland Dairy for the first 100 participants!
