Local Health Improvement Coalitions

Our work with Maryland's Local Health Improvement Coalitions (LHICs) went into high gear when the Maryland Department of Health released the Maryland Diabetes Action Plan for public comment. Since then, our technical assistance to LHICs has featured guidance on creating, implementing, and evaluating diabetes strategies at the local level.

Black Family-Focused Diabetes Control, Self-Management, and Prevention Program

The Center is working with FHI360 and the African American Health Program in Montgomery County, Maryland to implement a variety of diabetes prevention and management services and programs to Black families. Family members with prediabetes will join and attend the National Diabetes Prevention Program, and family members with diabetes will join and attend a diabetes self-management education and support (DSMES) program. The families will also receive support and guidance from a community health worker.

Family Approach to the National Diabetes Prevention Program

The Center is working with FHI360 and 6 sites in Maryland to see whether joining and participating in the National Diabetes Prevention Program with a close friend, family member, or co-worker affects recruitment and retention in the year-long program. The Center and FHI360 developed supplemental materials for the programs' lifestyle coaches to use throughout the program and encourage the paired approach.

PreventionLink

PreventionLink is a multi-county program addressing Southern Maryland’s need for higher quality public health programs. The Horowitz Center for Health Literacy helped the Prince George’s County Health Department learn about health literacy and chronic disease issues as the county builds a primary care network for chronic care patients in Southern Maryland. The chronic disease conditions are diabetes, high blood pressure, and high cholesterol. 

The overall project will focus on patients who use a high level of healthcare services. In Years 1-3 of the project, Center staff spoke with individuals who have enrolled, completed, or left the National Diabetes Prevention Program. The objective was to learn about and address barriers to completing the program. The Center also developed patient materials that addressed health literacy for diabetes.
